RcoM (Regulator of Carbon Monoxide Metabolism) is an engineered recombinant heme-containing protein being developed as an artificial oxygen carrier and blood substitute for the resuscitation of patients with hemorrhagic shock. Unlike traditional hemoglobin-based oxygen carriers (HBOCs), RcoM is designed with a significantly reduced nitric oxide (NO) dioxygenation rate, which helps mitigate the vasoconstrictive side effects typically caused by NO scavenging. Developed by researchers at the University of Maryland, University of Pittsburgh, and Wayne State University, RcoM demonstrates high thermal and chemical stability, ideal oxygen binding kinetics, and low autoxidation rates. In preclinical murine models of traumatic hemorrhagic shock, RcoM has shown the ability to support mean arterial pressure (MAP) effectively without the hypertensive effects associated with standard hemoglobin solutions.
