RCS-21 is a novel RNA-based therapeutic designed to prevent and treat inflammatory lung damage, particularly in the context of severe respiratory diseases such as COVID-19. The drug specifically targets macrophages in the lung by utilizing a unique delivery system that couples its active ingredient to trimannose, a sugar molecule recognized predominantly by macrophage receptors. This targeted approach enables efficient uptake of the drug into macrophages while minimizing exposure to other cell types. Mechanistically, RCS-21 functions as an inhibitor of microRNA 21 (miR-21), which is implicated in pathological activation of macrophages leading to inflammation and fibrosis during acute lung injury. Preclinical studies have demonstrated that inhaled RCS-21 reduces miR-21 levels, decreases fibrosis and inflammation, and prevents pulmonary dysfunction after acute lung damage in animal models. Early human trials are underway to assess safety and tolerability[1][4][5][6][8].
