Overview
Re-PUVA is a combination therapy for psoriasis that involves the oral administration of a retinoid derivative (such as etretinate or acitretin) together with systemic PUVA (psoralen plus ultraviolet A) photochemotherapy. The retinoid is typically given daily before and/or during the course of PUVA treatment. This approach aims to enhance the efficacy of PUVA, reduce the total UVA dose required, shorten treatment duration, and potentially lower long-term risks associated with high cumulative UVA exposure. Re-PUVA has shown particular benefit in patients who are resistant to standard PUVA therapy[5]. The mechanism involves both modulation of keratinocyte proliferation and differentiation by retinoids and DNA crosslinking/phototoxic effects from psoralen-activated UVA.
