Recamtruzumab (also known as SHR-A1811) is a next-generation antibody-drug conjugate (ADC) targeting the human epidermal growth factor receptor 2 (HER2). Developed by Jiangsu Hengrui Pharmaceuticals, it consists of a humanized anti-HER2 monoclonal antibody (trastuzumab) covalently linked to a potent topoisomerase I inhibitor payload, rezetecan (an exatecan derivative), via a protease-cleavable linker. The drug is designed to treat HER2-expressing or HER2-mutated solid tumors, particularly breast cancer, by binding to HER2 on the cell surface, undergoing internalization, and releasing the cytotoxic payload to induce DNA damage and apoptosis. It features a high drug-to-antibody ratio (DAR) of approximately 8 and exhibits a bystander killing effect, which enhances its efficacy in tumors with heterogeneous HER2 expression.
