Recaticimab (SHR-1209) is a humanized monoclonal IgG1 antibody that targets and inhibits proprotein convertase subtilisin/kexin type 9 (PCSK9), developed by Jiangsu Hengrui Pharmaceuticals. By binding to PCSK9 with high affinity, it prevents the degradation of low-density lipoprotein receptors (LDLR) on the surface of hepatocytes, thereby increasing the clearance of LDL cholesterol from the plasma. The antibody features an engineered Fc region to extend its half-life, supporting infrequent subcutaneous dosing schedules (every 1–3 months). In the context of the PISTIAS-3 clinical trial, recaticimab is administered in combination with standard therapy—which typically includes antiplatelet agents, statins, and blood pressure management—to evaluate its efficacy in improving 90-day functional outcomes in patients with acute ischemic stroke (AIS) associated with intracranial atherosclerotic stenosis (ICAS). The combination is hypothesized to provide rapid lipid-lowering and potential plaque-stabilizing effects beyond standard care alone.
