Recombinant human antithymocyte globulin (rhATG) is a fully human polyclonal antibody drug developed as an immunosuppressant for the prevention of acute rejection following organ transplantation, particularly kidney transplantation. Unlike traditional animal-derived ATGs (from rabbit or horse), rhATG is produced using recombinant technology from transgenic mice with a humanized immune system. The mechanism of action involves binding to multiple T-cell surface antigens (such as CD45, CD4, CD3, and CD5), leading to T-lymphocyte depletion via antibody-dependent cell cytotoxicity (ADCC) and other immune-mediated mechanisms. This results in suppression of the host immune response against transplanted tissues or organs. Recombinant production offers advantages such as scalability, consistent manufacturing quality, reduced risk of anti-animal immune reactions, and potentially improved safety and efficacy profiles compared to animal-derived products[2][4][5].
