Recombinant human insulin-like growth factor binding protein 7 (rhIGFBP7) is a biologic therapeutic candidate being investigated for the treatment of acute myeloid leukemia (AML). IGFBP7 is a secreted protein that belongs to the IGFBP superfamily and functions as a tumor suppressor. In AML, low expression of IGFBP7 is a characteristic of leukemic stem cells (LSCs) and is associated with chemotherapy resistance and poor clinical prognosis. rhIGFBP7 acts by inducing differentiation and apoptosis in AML cells and LSCs while sparing normal hematopoietic stem cells. Preclinical research indicates that rhIGFBP7 can sensitize AML cells to chemotherapy and reduce leukemic engraftment in xenograft models, suggesting its potential as a combination therapy to eliminate residual disease and prevent relapse.
