Recombinant human intestinal trefoil factor (rhITF), also known as recombinant human trefoil factor 3 (rhTFF3), is a biologic therapeutic consisting of a 59-amino acid secretory protein. Naturally produced by intestinal goblet cells, rhITF contains a characteristic trefoil disulfide loop structure that is essential for its biological activity. It functions by protecting the gastrointestinal mucosa from injury, stabilizing the mucus layer, and promoting epithelial cell migration and repair (restitution). Additionally, it exhibits anti-inflammatory properties by suppressing NF-κB signaling. rhITF has been primarily investigated as a topical oral spray for the prevention and treatment of chemotherapy-induced oral mucositis, particularly in patients receiving fluorouracil-based regimens for colorectal cancer. It has also shown potential in preclinical models for ulcerative colitis, necrotizing enterocolitis, and other gastrointestinal mucosal injuries.
