Recombinant human kallikrein-1 (DM199) is a biologic therapeutic being developed by DiaMedica Therapeutics. It is a recombinant version of the human tissue kallikrein-1 (KLK1) protein, a serine protease produced using Chinese hamster ovary (CHO) cells. The drug's mechanism of action involves the cleavage of low-molecular-weight kininogen to release kinins, such as bradykinin and kallidin. these kinins subsequently activate bradykinin B2 receptors, leading to localized vasodilation, improved microcirculation, and various tissue-protective effects, including anti-inflammatory, anti-apoptotic, anti-fibrotic, and antioxidative activities. DM199 is primarily being investigated for the treatment of acute ischemic stroke and chronic kidney disease, where it aims to restore the impaired kallikrein-kinin system to improve blood flow and reduce tissue damage.
