Recombinant human klotho is a biologic therapeutic consisting of the recombinant form of the human alpha-klotho protein, specifically the extracellular domain (amino acids 34-981). It functions as an essential co-receptor for fibroblast growth factor-23 (FGF-23), facilitating its binding to FGF receptors (primarily FGFR1c, FGFR3c, and FGFR4) to regulate phosphate and vitamin D metabolism. Beyond its role in mineral homeostasis, klotho exhibits pleiotropic effects, including the inhibition of insulin-like growth factor-1 (IGF-1) and Wnt signaling pathways, as well as anti-oxidative and anti-apoptotic properties. It is being investigated primarily for the treatment of kidney diseases, such as preventing the progression of acute kidney injury (AKI) to chronic kidney disease (CKD) and managing uremic cardiomyopathy. Development has been pursued by companies like Klotho Therapeutics.
