Recombinant human lactoferrin (rhLF) is a multifunctional protein produced through recombinant technology, often in hosts like rice, yeast (*Komagataella phaffii*), or fungi (*Trichoderma reesei*). It is being developed for various therapeutic applications, including the treatment of advanced renal cell carcinoma, prevention of antibiotic-associated diarrhea, and severe sepsis. Additionally, it is investigated for its effects on chronic inflammation and cognitive deterioration in the elderly, and for managing *Clostridium difficile* infection. rhLF also shows promise as a food ingredient for immune support in healthy adults, exhibiting anti-inflammatory, antibacterial, and antitumor properties. Its mechanism involves modulating immune responses, such as inhibiting the Toll-like receptor 4-nuclear factor-kappa B pathway, upregulating ERK 1 and 2 phosphorylation, and attenuating microglial activation.
