Overview

Recombinant human lecithin-cholesterol acyltransferase (rhLCAT), also known as MEDI6012, is a biologic enzyme replacement therapy developed by AstraZeneca (MedImmune) for the treatment of cardiovascular diseases. LCAT is a plasma enzyme that catalyzes the esterification of unesterified cholesterol into cholesteryl esters, a critical step in the maturation of high-density lipoprotein (HDL) particles and the process of reverse cholesterol transport (RCT). By supplementing endogenous LCAT activity, MEDI6012 aims to enhance the removal of cholesterol from atherosclerotic plaques and improve lipid profiles. It has been investigated in Phase 2 clinical trials, including the REAL-TIMI 63B study, for indications such as acute ST-segment elevation myocardial infarction (STEMI) and stable atherosclerotic cardiovascular disease. Although it successfully increased HDL-cholesterol levels in clinical studies, it did not meet primary endpoints for reducing infarct size or regressing noncalcified coronary plaque in some trials.
