Recombinant human nerve growth factor (rhNGF) is a biologic neurotrophin that was investigated for the treatment of HIV-associated sensory neuropathy. It acts as an agonist for the High affinity nerve growth factor receptor (TrkA) located on small-diameter sensory neurons in the peripheral nervous system. By stimulating these neurons, rhNGF aims to promote the survival and regeneration of damaged nerve fibers, addressing the underlying pathology of painful neuropathy. The National Institute of Allergy and Infectious Diseases (NIAID) sponsored a Phase II clinical trial (ACTG 291) to evaluate its safety and efficacy. Although the study showed positive trends in pain reduction and sensory improvement, the clinical development was hindered by side effects such as injection site pain.
