Recombinant human plasmin (rhPlasmin) is a direct-acting thrombolytic agent produced through recombinant DNA technology, designed to dissolve blood clots by directly degrading fibrin. Unlike indirect thrombolytics like tissue plasminogen activator (tPA), which require the conversion of endogenous plasminogen to plasmin, rhPlasmin is the active enzyme itself. This direct mechanism allows for rapid clot lysis upon contact. A significant pharmacological advantage of rhPlasmin is its susceptibility to rapid neutralization by alpha-2-antiplasmin in the systemic circulation; this localization of activity helps minimize systemic fibrinogen depletion and reduces the risk of distant bleeding complications when administered locally. Developed primarily by Talecris Biotherapeutics (now Grifols), rhPlasmin has been investigated in clinical trials for the treatment of acute peripheral arterial occlusion (aPAO) and the restoration of patency in occluded central venous catheters.
