Recombinant human proteoglycan 4 (rhPRG4), also known as recombinant human lubricin, is a large, highly glycosylated mucin-like glycoprotein developed by Lubris Bio. It is designed to mimic the function of endogenous lubricin, which is naturally present in the tear film and on the ocular surface to provide boundary lubrication and reduce friction between the eyelid and the cornea. In conditions like Sjögren's syndrome and dry eye disease, lubricin expression is often downregulated, leading to increased mechanical friction and subsequent inflammation. Beyond its mechanical lubricating properties, rhPRG4 has been shown to interact with receptors such as CD44 and Toll-like receptors (TLR2 and TLR4), providing anti-inflammatory effects by inhibiting pro-inflammatory signaling. It is currently being evaluated in clinical trials as a topical ophthalmic solution to alleviate symptoms and signs of ocular surface damage in patients with Sjögren's-related dry eye disease.
