Recombinant neutrophil inhibitory factor and hirulog hybrid, also called **TNHH**, is an investigational recombinant hybrid glycoprotein developed for acute ischemic stroke. It combines a neutrophil inhibitory factor-derived component that binds the neutrophil integrin CD11b/CD18 with a hirulog-derived direct thrombin-inhibitory component, aiming to simultaneously limit neutrophil adhesion and migration and reduce thrombin-mediated coagulation at ischemic thrombi. A first-in-human randomized, placebo-controlled, single-ascending-dose study in healthy volunteers evaluated intravenous TNHH at 0.05-1.8 mg/kg; it showed high CD11b/CD18 occupancy at doses above 0.2 mg/kg and dose-related prolongation of thrombin time. The program has been associated with West China Hospital, Chongqing Fagen Biomedical, and Shandong New Time Pharmaceutical.
