Overview

Recombinant soluble human CD4 is a genetically engineered, truncated form of the human CD4 receptor produced via recombinant DNA technology. This soluble protein mimics the external domain of the natural CD4 receptor and is capable of binding to the HIV-1 envelope glycoprotein gp120, thereby blocking the virus's ability to attach to and infect CD4+ T lymphocytes. As a result, it acts as a viral entry inhibitor. Preclinical and early clinical studies indicated that recombinant soluble human CD4 can neutralize laboratory-adapted HIV-1 in vitro and has shown safety and some antiviral activity in patients with HIV/AIDS. However, its ability to neutralize clinical (primary) HIV-1 isolates is limited due to the much higher concentrations required. Recombinant soluble human CD4 was developed as an experimental therapeutic and was tested primarily in the late 1980s and early 1990s for HIV/AIDS, but further clinical development was largely discontinued due to practical limitations and modest clinical efficacy.
