The recombinant soluble PSMA (rsPSMA) vaccine is a protein subunit biologic vaccine developed for the treatment of advanced prostate cancer, including metastatic castrate-resistant prostate cancer (mCRPC). The active ingredient is a recombinant soluble form of the prostate-specific membrane antigen (PSMA), a glycoprotein that is highly overexpressed on the surface of prostate cancer cells. The vaccine utilizes a noncovalent dimer of the rsPSMA protein, which preserves its native conformation and enzymatic activity—a feature critical for inducing antibodies that can effectively cross-react with PSMA-expressing tumor cells. Formulated with Alhydrogel (aluminum hydroxide) as an immunological adjuvant, the vaccine is designed to stimulate the immune system to produce both antibodies and cancer-fighting cells against the tumor. Development was supported by NIH-funded research (grant R44-CA091746-02) and progressed into Phase I clinical trials before the program was terminated.
