Reconstituted high-density lipoproteins (rHDL) is an investigational biologic being developed by the University of Adelaide's Vascular Research Centre and the South Australian Health and Medical Research Institute (SAHMRI) for the treatment of diabetic foot ulcers and impaired wound healing. The formulation consists of discoidal nanoparticles composed of apolipoprotein A-I (apoA-I) complexed with the phospholipid 1-palmitoyl-2-linoleoyl-phosphatidylcholine (PLPC). Administered topically, rHDL targets the scavenger receptor class B type I (SR-BI) and modulates the PDK4/pyruvate dehydrogenase complex (PDC) axis to rescue diabetes-impaired angiogenesis, reduce inflammation, and promote wound closure. It has been evaluated in a Phase 1/2 clinical trial in Australia for acute post-ray-amputation wounds in patients with diabetes.
