Overview

This is a combination therapy consisting of four components commonly used in the management of major hemorrhage, trauma, or perioperative bleeding. The components are: - **Red blood cells** (RBCs): Cellular blood component responsible for oxygen transport to tissues. - **Tranexamic acid**: A synthetic antifibrinolytic agent that inhibits the breakdown of fibrin clots by blocking lysine binding sites on plasminogen molecules, thereby reducing bleeding. - **Fibrinogen concentrate**: A plasma-derived or recombinant product providing concentrated fibrinogen, a key protein in clot formation and stabilization. - **Crystalloids**: Intravenous fluids (such as saline or Ringer's lactate) used for volume resuscitation and maintenance of intravascular volume. This combination is not a single pharmaceutical product but rather an evidence-based protocolized approach to treating acute severe bleeding. It aims to restore circulating volume (crystalloids), improve oxygen delivery (red blood cells), promote hemostasis by inhibiting fibrinolysis (tranexamic acid), and replenish depleted clotting factors—especially fibrinogen—to support effective coagulation.
