Overview

This regimen is a specific reduced-intensity conditioning (RIC) protocol developed by New York Medical College for pediatric and young adult patients undergoing allogeneic hematopoietic stem cell transplantation (HSCT). It is primarily designed for patients with Fanconi anemia or other high-risk conditions including leukemia, lymphoma, bone marrow failure, and various non-malignant disorders. The protocol combines immunoablative and myelosuppressive agents—specifically fludarabine, cyclophosphamide, and horse antithymocyte globulin (ATG)—with a single fraction of total body irradiation (TBI). This conditioning is followed by the infusion of CD34+ selected stem cells, a technique intended to provide a pure population of hematopoietic progenitors while depleting T-cells to reduce the incidence and severity of graft-versus-host disease (GVHD) in recipients of partially matched or unrelated donor grafts.
