Reduced Intensity Conditioning-New York Medical College-Hodgkin lymphoma refers to a specific clinical trial protocol (NCT02588612) designed for children, adolescents, and young adults with high-risk or relapsed/refractory CD30+ Hodgkin lymphoma. The regimen involves a reduced intensity conditioning (RIC) phase using chemotherapy agents such as fludarabine, busulfan, melphalan, or cyclophosphamide to prepare the patient for an allogeneic stem cell transplantation (AlloSCT). Following the transplant, patients receive brentuximab vedotin (Adcetris) as a targeted consolidation therapy. Brentuximab vedotin is an antibody-drug conjugate (ADC) that binds to CD30 on the surface of Hodgkin lymphoma cells, internalizes, and releases the potent microtubule-disrupting agent monomethyl auristatin E (MMAE), leading to cell cycle arrest and apoptosis. This multi-modal approach aims to leverage the graft-versus-lymphoma effect of the transplant while minimizing toxicity through reduced-intensity conditioning and enhancing efficacy with targeted biologic therapy.
