Overview

Reesterified long-chain n-3 polyunsaturated fatty acids (reesterified LC n-3 PUFAs) are a specific formulation of omega-3 fatty acids in which the free fatty acid or ethyl ester forms of eicosapentaenoic acid (EPA), docosahexaenoic acid (DHA), and sometimes docosapentaenoic acid (DPA) are enzymatically or chemically converted back into triglycerides after purification. This process yields a product with a higher concentration of EPA and DHA in the triglyceride form compared to natural fish oil. The main mechanism of action involves incorporation into cell membranes and subsequent metabolism to bioactive lipid mediators called eicosanoids, which have anti-inflammatory, anti-thrombotic, anti-arrhythmic, and lipid-lowering effects[6][7][4]. These actions contribute to reduced plasma triglycerides, decreased risk of fatal coronary heart disease events[6], stabilization of atherosclerotic plaques through anti-inflammatory effects[4], modulation of cardiac electrophysiology leading to fewer arrhythmias[4], and altered platelet aggregation reducing thrombosis risk[6]. Reesterification is thought to improve bioavailability compared with ethyl ester forms. Primary indications include hypertriglyceridemia management as an adjunct to diet for lowering very high triglycerides; secondary prevention in cardiovascular disease; potential roles in inflammatory conditions such as rheumatoid arthritis; maternal-fetal health; and possible benefits for cognitive function and other chronic diseases[1][2][5].
