Refined indigo naturalis is a processed extract derived from indigo naturalis (also known as qing dai), which has been used in traditional Chinese medicine for centuries. The refined version contains a higher concentration of active ingredients, particularly indirubin, compared to the crude form. ## Description Refined indigo naturalis is a purified extract from indigo naturalis, which is derived from plants such as Baphicacanthus cusia, Indigofera tinctoria, Isatis indigotica, Polygonum tinctorium, and Strobilanthes cusia. The refinement process increases the concentration of active components, particularly indirubin, which can comprise at least 55% of the refined extract according to some patents[10]. The extract has demonstrated significant anti-inflammatory, anti-proliferative, and anti-angiogenic properties. It works by regulating multiple signaling pathways including TAK1, JAK3/STAT3, Wnt/β-catenin, Akt/PKB, FAK, and AP-1/c-Jun pathways[2]. These mechanisms make it particularly effective for treating inflammatory conditions such as psoriasis and ulcerative colitis. Clinical studies have shown that refined indigo naturalis is more effective than crude indigo naturalis for treating psoriasis, with a more favorable side effect profile[4]. It has been formulated into various preparations including ointments and oils for topical application. ## Active Components The main active components of refined indigo naturalis include: - Indigo - Indirubin (the most potent component) - Tryptanthrin - Isatin - Isoindigo These compounds, particularly indirubin, are responsible for the therapeutic effects through their ability to inhibit inflammatory cytokines and regulate cell proliferation[7][8]. ## Clinical Applications Refined indigo naturalis has shown efficacy in treating: 1. **Psoriasis** - Multiple studies demonstrate significant improvement in psoriatic symptoms with topical application[4][5][9] 2. **Nail psoriasis** - Particularly effective when formulated in an oil vehicle[5][9] 3. **Ulcerative colitis** - Shows promise as both an induction and maintenance therapy[7] ## Safety Profile While generally well-tolerated, particularly in topical applications, refined indigo naturalis may cause: - Mild liver dysfunction - Abdominal pain - Headache - Pruritus, rash, and erythema (with topical use) - Rare cases of pancreatitis, colitis, and pulmonary hypertension (with oral use)[1][7] The risk of adverse events appears to increase with treatment duration, particularly with oral administration for conditions like ulcerative colitis[7].
