Regimen 2, Course 1 is a multi-agent combination chemotherapy protocol developed by Baylor College of Medicine for the treatment of brain tumors in infants (typically under 3 years of age). This regimen is part of a pilot clinical study designed to evaluate the safety and maximum tolerated dose of intensive chemotherapy, often used in conjunction with intrathecal agents and conformal radiation to delay or eliminate the need for high-dose craniospinal irradiation. The protocol is structured into three distinct phases: Cycle C1 and Cycle C2, which involve the administration of intravenous cyclophosphamide (a DNA alkylating agent), vincristine (a microtubule inhibitor), and mesna (a uroprotective agent), followed by G-CSF (filgrastim) support; and Cycle D, which consists of oral etoposide, a topoisomerase II inhibitor. This sequential approach aims to maximize tumor control while managing systemic toxicities in a vulnerable pediatric population.
