REGN4336 is a bispecific monoclonal antibody designed to target both the prostate-specific membrane antigen (PSMA), which is highly expressed on prostate cancer cells, and the CD3 antigen found on T lymphocytes. By binding simultaneously to PSMA and CD3, REGN4336 acts as a T-cell engager that brings cytotoxic T-cells into close proximity with PSMA-expressing tumor cells, facilitating targeted immune-mediated killing of these cancer cells. This mechanism enables activation of cytotoxic T-cells independent of co-stimulation pathways, resulting in robust antitumor activity. The drug is being developed by Regeneron Pharmaceuticals for the treatment of advanced prostate carcinoma, specifically metastatic castration-resistant prostate cancer (mCRPC). It is currently being evaluated in phase 1/2 clinical trials as monotherapy and in combination with other immunotherapies such as cemiplimab or nezastomig[1][3][4][7].
