Overview

REGN5668 is an investigational human bispecific antibody developed by Regeneron Pharmaceuticals. It targets MUC16 (a tumor-associated antigen highly expressed in ovarian and other gynecologic cancers) and CD28 (a costimulatory receptor on T cells). By binding to both MUC16 on tumor cells and CD28 on T cells, REGN5668 is designed to bridge these cell types, providing a costimulatory signal that activates T lymphocytes against the cancer. This mechanism aims to enhance anti-tumor immune responses through antibody-dependent cell cytotoxicity and direct T-cell stimulation. REGN5668 is being studied primarily for recurrent ovarian cancer, fallopian tube cancer, primary peritoneal cancer, and endometrial cancer—both as monotherapy and in combination with other agents such as cemiplimab (an anti-PD-1 monoclonal antibody) or REGN4018 (a MUC16xCD3 bispecific antibody)[1][4][5][9].
