REGN9165 is an investigational bispecific monoclonal antibody developed by Regeneron Pharmaceuticals that simultaneously targets the Angiopoietin-1 receptor (TIE2) and the T-cell costimulatory receptor CD28. TIE2 is predominantly expressed on the vascular endothelium and certain tumor-associated macrophages within the tumor microenvironment, serving as an effective anchor for localizing the antibody. REGN9165 is designed to provide a 'Signal 2' costimulatory signal to T-cells only upon binding to TIE2-expressing cells, thereby avoiding the systemic toxicity associated with non-targeted CD28 activation. This mechanism is intended to synergize with 'Signal 1' therapies, such as PD-1 inhibitors (e.g., cemiplimab) or CD3-based bispecific antibodies, to promote robust anti-tumor T-cell responses. It is currently being evaluated in Phase 1 clinical trials for patients with advanced solid tumors.
