Overview

Regorafenib + nivolumab is an investigational combination therapy consisting of two distinct agents: regorafenib, a multi-kinase inhibitor, and nivolumab, a programmed death 1 (PD-1) immune checkpoint inhibitor. Regorafenib inhibits multiple protein kinases involved in tumor angiogenesis (including VEGFR), oncogenesis, and the tumor microenvironment. Nivolumab is a monoclonal antibody that blocks PD-1 on T cells, thereby enhancing anti-tumor immune responses. The rationale for combining these drugs is that regorafenib may modulate the tumor microenvironment to overcome resistance to immunotherapy by reducing immunosuppressive myeloid cells and promoting anti-tumor immunity, while nivolumab activates T-cell mediated cytotoxicity against cancer cells. This combination has been studied primarily in patients with microsatellite stable/mismatch repair proficient metastatic colorectal cancer (MSS/pMMR mCRC), unresectable hepatocellular carcinoma, advanced gastric cancer, and other solid tumors[1][2][3][5][6][8]. Both drugs are individually approved for various cancers but not as this specific combination.
