Overview

Reldesemtiv is an investigational small molecule drug developed as a fast skeletal muscle troponin activator (FSTA). It selectively binds to the fast skeletal muscle troponin complex and sensitizes it to calcium, thereby increasing skeletal muscle contractility in response to neuronal input. This mechanism is intended to improve muscle function in diseases characterized by muscle weakness or fatigue, such as amyotrophic lateral sclerosis (ALS) and spinal muscular atrophy (SMA)[6][8]. Reldesemtiv was developed by Cytokinetics and advanced through multiple clinical trials for ALS and SMA. In ALS, it aimed to slow disease progression by enhancing the force of muscle contraction despite reduced nerve signaling[5][7]. However, development for ALS was discontinued after phase 3 trials failed to demonstrate significant efficacy[2][5].
