Overview

Remifentanil is a potent, ultra short–acting synthetic opioid analgesic used primarily as an adjunct to anesthesia for pain relief during surgery. It acts as a selective agonist at the mu-opioid receptor, producing rapid onset and offset of analgesia due to its unique metabolism by nonspecific blood and tissue esterases. This pharmacokinetic profile allows for precise control over sedation and quick post-operative recovery, making it suitable for procedures requiring rapid changes in anesthetic depth or fast emergence from anesthesia. Remifentanil is administered intravenously, often via continuous infusion, and is commonly used in general anesthesia (including TIVA/TCI techniques), sedation during intensive care or procedural settings, and occasionally for labor analgesia when epidural is not feasible. Its use requires careful monitoring due to risks of respiratory depression typical of opioids[1][2][4][5][6].
