Overview

Remifentanil + nicardipine is an investigational intravenous combination regimen used to induce and maintain controlled hypotension during surgery, particularly in otolaryngological (ear, nose, and throat) procedures. Remifentanil is an ultra-short-acting synthetic opioid that acts as a mu-opioid receptor agonist, providing rapid-onset analgesia and contributing to blood pressure reduction. Nicardipine is a second-generation dihydropyridine calcium channel blocker that induces potent vasodilation by inhibiting calcium influx into vascular smooth muscle. The co-administration of these agents aims to optimize surgical field visibility and minimize blood loss by precisely titrating mean arterial pressure. This regimen is currently being evaluated by Dicle University to assess its impact on advanced hemodynamic parameters, such as cardiac output and systemic vascular resistance, compared to remifentanil monotherapy.
