Overview

Remlifanserin is an oral small-molecule selective serotonin 5-HT2A receptor inverse agonist/antagonist being developed by Acadia Pharmaceuticals for the treatment of dementia-related psychosis, with a primary focus on Alzheimer’s disease psychosis and also Lewy body disease psychosis.[2][11][13][14] Designed as an improved follow-up to pimavanserin, it exhibits higher potency and markedly greater selectivity for 5-HT2A over 5-HT2C and very low affinity for 5-HT2B, reducing off‑target and cardiac liabilities such as QT prolongation.[2][3][5][11] By selectively reducing pathological 5-HT2A signaling without dopaminergic blockade, remlifanserin aims to control hallucinations and delusions in dementia while minimizing motor and cognitive side effects common to traditional antipsychotics.[2][11][14]
