Remzistotug (GSK4074386) is a humanized monoclonal antibody of the IgG4 isotype designed to target the Lymphocyte-activation gene 3 (LAG-3) protein, an inhibitory receptor expressed on the surface of effector T cells and regulatory T cells. Developed by GSK, remzistotug acts as an immune checkpoint inhibitor by blocking the interaction between LAG-3 and its ligands, such as major histocompatibility complex (MHC) class II molecules. This blockade prevents inhibitory signaling that typically leads to T-cell exhaustion and suppression of the immune response within the tumor microenvironment. By restoring T-cell activation and proliferation, remzistotug enhances anti-tumor immunity. It is currently being evaluated in early-phase clinical trials both as a monotherapy and in combination with other immunotherapies, such as the PD-1 inhibitor dostarlimab, for the treatment of various advanced solid tumors.
