Overview

Reovirus R124 is a wildtype mammalian orthoreovirus (MRV) of the Type 3 Dearing (T3D) lineage, currently under investigation as an oncolytic virotherapy. Developed within the multi-center Oncolytic Viro-Immunotherapy (OVIT) consortium, primarily at the Leiden University Medical Center (LUMC), R124 selectively infects and replicates in malignant cells, utilizing Junction Adhesion Molecule A (JAM-A) as its primary entry receptor. The therapeutic mechanism involves direct viral-mediated oncolysis and the induction of immunogenic cell death (ICD), which is marked by the release of damage-associated molecular patterns (DAMPs) such as HMGB1 and the upregulation of calreticulin and HSP90. These events stimulate a robust anti-tumor immune response, including the activation of natural killer (NK) cells and dendritic cells, and the secretion of pro-inflammatory cytokines like IFNβ and CXCL10. Preclinical studies have demonstrated its efficacy in human bladder cancer models, including patient-derived 3D cultures and ex-vivo tumor tissue slices.
