repaglinide + voglibose

Combose is an oral fixed-dose combination of the meglitinide analog repaglinide and the alpha-glucosidase inhibitor voglibose, developed by ADN Life Sciences for the management of type 2 diabetes mellitus. Repaglinide acts as an insulin secretagogue by binding to and closing ATP-sensitive potassium (KATP) channels (specifically the SUR1 subunit) on pancreatic beta cells, which depolarizes the cell membrane, opens voltage-gated calcium channels, and triggers insulin release. Voglibose competitively inhibits intestinal alpha-glucosidase enzymes (such as maltase-glucoamylase and sucrase-isomaltase) in the brush border of the small intestine, delaying carbohydrate digestion and glucose absorption. Together, these complementary mechanisms of action effectively target postprandial hyperglycemia and improve overall glycemic control in patients with type 2 diabetes.
