Overview

Repository corticotropin injection is a sterile, prolonged-release preparation of the natural form of adrenocorticotropic hormone (ACTH), typically derived from porcine pituitary glands and formulated in gelatin. It is classified as a hormone and used to treat a range of corticosteroid-responsive conditions, including infantile spasms in children under 2 years old, acute exacerbations of multiple sclerosis, various rheumatic disorders (such as rheumatoid arthritis, psoriatic arthritis, ankylosing spondylitis), collagen diseases (like systemic lupus erythematosus and polymyositis), dermatologic diseases (including Stevens-Johnson syndrome and psoriasis), allergic states (such as serum sickness), ophthalmic diseases (e.g., optic neuritis), respiratory diseases like sarcoidosis, nephrotic syndrome with proteinuria, and certain edematous states. Its mechanism involves stimulating the adrenal cortex to increase endogenous production of cortisol and other glucocorticoids; this results in immunosuppressive effects that reduce inflammation. The precise mechanism for some indications—such as infantile spasms—is not fully understood[1][4][5][6][10].
