Overview

Resomelagon (AP1189) is an orally available, once-daily small molecule selective melanocortin receptor agonist that targets the melanocortin type 1 (MC1R) and type 3 (MC3R) receptors. Developed by SynAct Pharma, it is designed to promote the resolution of inflammation by activating endogenous pathways that dampen inflammatory responses and facilitate tissue repair without suppressing the immune system. In the context of idiopathic membranous nephropathy (iMN) and nephrotic syndrome, resomelagon aims to protect podocytes, which express MC1R, thereby preserving glomerular structure and reducing severe proteinuria. It is currently being evaluated as an add-on therapy to standard-of-care treatments such as ACE inhibitors or angiotensin II receptor blockers (ARBs).
