Retlirafusp alfa + apatinib + nab-paclitaxel is a combination therapy being evaluated for the treatment of advanced gastric and gastroesophageal junction (GEJ) cancers. Retlirafusp alfa (SHR-1701) is a bifunctional fusion protein that targets both programmed death-ligand 1 (PD-L1) and transforming growth factor-beta receptor II (TGF-βRII), aiming to reverse immune suppression in the tumor microenvironment. Apatinib is a selective tyrosine kinase inhibitor of vascular endothelial growth factor receptor-2 (VEGFR-2), which inhibits tumor-induced angiogenesis. Nab-paclitaxel is an albumin-bound formulation of the cytotoxic agent paclitaxel, which disrupts microtubule function to inhibit cell division. This combination is specifically being studied as a second-line treatment for patients with HER2-negative advanced gastric or GEJ adenocarcinoma who have previously failed immunotherapy-containing regimens.
