Overview

SHR-1701 + SOX is an investigational combination regimen being evaluated for the perioperative (neoadjuvant and adjuvant) treatment of locally advanced resectable Siewert type II adenocarcinoma of the esophagogastric junction (EGJ). The regimen consists of **SHR-1701** (retlirafusp alfa), a novel bifunctional fusion protein, and **SOX**, a standard chemotherapy doublet. SHR-1701 is composed of a humanized anti-PD-L1 monoclonal antibody fused to the extracellular domain of the TGF-β receptor II (TGF-βRII). This dual-targeting mechanism allows it to simultaneously block the PD-1/PD-L1 immune checkpoint and sequester all three isoforms of TGF-β (TGF-β1, TGF-β2, and TGF-β3), thereby reversing TGF-β-mediated immunosuppression and enhancing anti-tumor T-cell activity. SOX chemotherapy consists of **S-1** (an oral fluoropyrimidine combination of tegafur, gimeracil, and oteracil) and **oxaliplatin** (a platinum-based cytotoxic agent). The combination aims to leverage the synergistic potential of immune checkpoint inhibition, TGF-β signaling blockade, and cytotoxic chemotherapy to improve pathological complete response rates and survival in gastroesophageal cancers.
