Overview

Retlirafusp alfa + TP-AC is a combination therapy regimen being investigated for the treatment of triple-negative breast cancer (TNBC). Retlirafusp alfa (SHR-1701) is a bifunctional fusion protein composed of a monoclonal antibody against programmed death-ligand 1 (PD-L1) fused to the extracellular domain of the transforming growth factor-beta receptor type II (TGF-βRII). This dual mechanism aims to simultaneously block the PD-1/PD-L1 immunosuppressive pathway and neutralize TGF-β in the tumor microenvironment, which is often associated with immune exclusion and drug resistance. The TP-AC component refers to a sequential chemotherapy regimen consisting of a taxane (typically nab-paclitaxel) and a platinum agent (carboplatin), followed by adriamycin (doxorubicin) and cyclophosphamide. This combination is primarily being evaluated in Phase 2 clinical trials for early-stage and advanced TNBC to improve pathological complete response rates and survival outcomes.
