RevCAR-E5B9 is a component of a modular, switchable chimeric antigen receptor (CAR) T-cell platform known as the RevCAR system. Unlike traditional CAR T-cells that directly bind to tumor antigens, RevCAR-E5B9 T-cells express a "reverse" CAR that recognizes a specific peptide epitope, E5B9 (a 10-amino acid sequence derived from the human nuclear La/SS-B protein). To target tumor cells, a bispecific adapter molecule called a RevTM (Reverse Target Module) is required; one end of the RevTM binds the E5B9 epitope on the T-cell, while the other binds a tumor-associated antigen (such as CD33, CD123, or PSMA). This design allows for precise control over T-cell activity, as the therapy can be switched "on" or "off" by administering or withholding the RevTM, potentially reducing off-tumor toxicity and cytokine release syndrome. The construct is a second-generation CAR containing CD28 costimulatory and CD3 zeta signaling domains.
