Reverse hybrid regimen is a combination antibiotic therapy used for the eradication of Helicobacter pylori infection. It is a simplified, one-step, two-phase treatment protocol that involves administering a proton pump inhibitor (PPI) plus amoxicillin for the full duration (typically 12–14 days), with clarithromycin and metronidazole added only during the initial 7 days. This approach reverses the sequence of drug administration compared to standard "hybrid" regimens, aiming to simplify dosing and improve patient adherence. The mechanism of action involves inhibition of gastric acid secretion by PPIs and bactericidal effects from amoxicillin, clarithromycin (a macrolide), and metronidazole (a nitroimidazole). Reverse hybrid therapy has demonstrated high eradication rates for H. pylori—including in populations with moderate resistance to clarithromycin or metronidazole—and similar or fewer adverse events compared to other regimens such as standard triple or concomitant therapies[1][2][4][5].
