**Revumenib + chemotherapy** is an investigational combination regimen consisting of revumenib, an oral menin inhibitor, together with various standard chemotherapeutic agents. Revumenib inhibits the interaction between menin and mixed-lineage leukemia 1 (KMT2A, formerly MLL), disrupting a critical oncogenic pathway in acute leukemias harboring KMT2A rearrangements or NPM1 mutations. The combination is primarily developed for relapsed or refractory (R/R) acute lymphoblastic leukemia (ALL) and acute myeloid leukemia (AML)—especially in patients with KMT2A rearrangements or NPM1 mutations. Chemotherapy backbones studied with revumenib include cytarabine, daunorubicin, vincristine, prednisone or prednisolone, calaspargase pegol, methotrexate, hydrocortisone, fludarabine, and midostaurin (for FLT3-mutated AML)[1][5]. The combination aims to improve remission rates and outcomes in young children and adults for whom prognosis is otherwise poor.
