Drug intelligence / Profile preview
The combination of revumenib (Revuforj), venetoclax (Venclexta), and decitabine/cedazuridine (Inqovi)—collectively known as the SAVE regimen—is an all-oral triplet therapy developed for the treatment of acute myeloid leukemia (AML), particularly subtypes harboring menin-dependent genetic alterations such as KMT2A rearrangements, NPM1 mutations, or NUP98 rearrangements. Revumenib is a first-in-class small molecule inhibitor of the menin-KMT2A interaction, which disrupts the leukemogenic transcriptional program. Venetoclax is a selective B-cell lymphoma-2 (BCL-2) inhibitor that promotes apoptosis in leukemia cells. Decitabine/cedazuridine is an oral hypomethylating agent combination, where cedazuridine inhibits cytidine deaminase to prevent the degradation of decitabine, allowing for therapeutic oral delivery of the hypomethylating agent. Preclinical studies have demonstrated strong synergy between menin inhibition and BCL-2 inhibition. In the phase 1/2 SAVE trial, this all-oral triplet demonstrated high rates of composite complete remission and deep measurable residual disease (MRD) negativity in patients with relapsed or refractory AML, serving as an effective bridge to allogeneic hematopoietic stem cell transplantation.
