RFB4 is a murine monoclonal antibody specifically directed against human CD22, a B cell-restricted cell surface glycoprotein. As an unconjugated antibody, RFB4 serves as the targeting component in several immunotoxins and antibody-drug conjugates. The best studied toxin conjugate, RFB4-dgA or RFB4-SMPT-dgA, chemically links the deglycosylated A chain of ricin (dgA) to RFB4 in order to target and kill CD22-positive B cell malignancies. The conjugate binds to CD22, is internalized, and delivers the ricin A chain payload to inhibit protein biosynthesis, leading to apoptosis of the malignant cell. RFB4-based immunotoxins have demonstrated activity in preclinical models and early phase clinical trials, mainly for CD22-positive lymphomas and leukemias. Capillary leak or vascular leak syndrome is a dose-limiting toxicity. The originator and major developer was the National Cancer Institute[1][2][4].
