rfhSP-D is a recombinant fragment of human surfactant protein D (SP-D) being developed by University College London for the prevention of bronchopulmonary dysplasia (BPD) and neonatal chronic lung disease in preterm infants. As a collectin protein, it functions as part of the lung's innate immune system, aiding in the clearance of pathogens and modulating the inflammatory response. The drug works by down-regulating pro-inflammatory signaling pathways, such as the Toll-like receptor 4 (TLR4) pathway, thereby reducing the recruitment of inflammatory cells and oxidative stress in the developing lungs. It is currently being evaluated in the Phase 1 RESPONSE trial via endotracheal administration in infants born before 30 weeks gestation with respiratory distress syndrome.
