Overview

RFT5-dgA-depleted donor T-cell infusion is an adoptive cell therapy product developed by Baylor College of Medicine for use in patients undergoing haploidentical stem cell transplantation. The therapy involves the ex vivo treatment of donor lymphocytes with RFT5-dgA, an immunotoxin composed of a murine monoclonal antibody (RFT5) targeting the CD25 antigen (IL-2 receptor alpha chain) conjugated to a deglycosylated ricin A chain (dgA). During the manufacturing process, donor T cells are co-cultured with recipient cells to induce the activation of alloreactive (recipient-reactive) T cells, which then express CD25. The RFT5-dgA immunotoxin selectively binds to these activated cells and delivers the ricin A chain payload, which inhibits ribosomal protein synthesis and induces cell death. This selective depletion aims to reduce the risk of severe Graft-versus-Host Disease (GVHD) while preserving non-alloreactive T cells that provide essential anti-viral and anti-tumor immunity in the post-transplant period.
