Overview

RG6351 (also known as RO7446603) is a novel multivalent Tie2 agonist developed by Roche for the treatment of diabetic macular edema (DME). The drug is engineered as a Fab-PEG-hexamer conjugate, featuring six antibody Fab fragments linked to a polyethylene glycol (PEG) core. This unique hexameric structure is designed to induce clustering and activation of the Tie2 receptor (TEK tyrosine kinase) on endothelial cells, effectively mimicking the vascular-stabilizing activity of the natural ligand Angiopoietin-1. By activating the Tie2 pathway, RG6351 aims to restore the blood-retinal barrier, reduce vascular permeability, and suppress inflammatory processes that contribute to vision loss in DME. It is currently being evaluated in Phase 2 clinical trials as an intravitreal injection.
