Overview

RH genotype-matched D-positive red blood cells are a specialized blood product intervention developed by the Children's Hospital of Philadelphia for the treatment of chronically transfused patients with sickle cell disease. This approach targets patients who are genetically D-positive but have developed anti-D antibodies, a condition that usually necessitates lifelong transfusion with D-negative blood. By using precise RHD genotyping to match donor units (specifically from Black donors to ensure better overall antigen matching) with recipients, the protocol aims to safely provide D-positive blood without restimulating anti-D antibodies. This strategy is intended to improve the management of Rh alloimmunization, optimize the use of valuable donor units, and alleviate the strain on Rh-negative blood inventories.
